Monopolar Radiofrequency

Monopolar Radiofrequency (RF) is a non-invasive procedure that uses controlled radiofrequency energy to heat the deeper layers of the skin — the dermis and subcutaneous tissue — to stimulate the body’s natural collagen and elastin remodelling response. Unlike bipolar and multipolar RF devices, which confine their energy between two or more electrodes positioned close together on the skin’s surface, monopolar RF delivers energy through a single handpiece electrode and grounds it through a return pad placed elsewhere on the body. This configuration allows the energy to penetrate more deeply and uniformly into the target tissue, reaching the dermis and deeper layers relevant for structural skin tightening and body contouring.

The treatment is delivered by our medical doctors and is customised to the individual’s skin condition, anatomy, and goals. Suitability is assessed at consultation.

What is Monopolar Radiofrequency?

Monopolar Radiofrequency (MRF) is a non-invasive energy-based treatment that uses alternating electrical current at radiofrequency (typically 1 MHz) to heat tissue beneath the skin’s surface. The electromagnetic energy causes oscillation of water molecules in the target tissue, generating controlled heat at depths relevant to the dermis and subcutaneous tissue — without disrupting the skin surface.

How monopolar differs from other RF configurations

Radiofrequency devices are available in several electrode configurations — monopolar, bipolar, multipolar, and fractional. In a monopolar configuration, the active treatment electrode is in the handpiece applied to the skin, and a grounding return pad is placed on a separate body surface (commonly the lower back or thigh). The electrical current must travel from the handpiece, through the body, to the return pad — and it is along this path that the tissue is heated. This results in deeper energy penetration than bipolar systems (where the current path is confined between two electrodes positioned just millimetres apart on the skin surface).

The deeper penetration of monopolar RF is why it is used for structural skin tightening and body contouring applications that require heating the deep dermis, subcutaneous tissue, and fibrous septae — not just the surface layers.

How Does Monopolar Radiofrequency Work?

Immediate Collagen Contraction

When the dermis is heated to approximately 60–70°C, the triple-helix structure of existing collagen fibres contracts immediately — a direct physical response to heat. This immediate collagen contraction produces a subtle initial tightening effect that can be appreciated relatively soon after treatment, before the longer-term remodelling effects develop.

Collagen and Elastin Remodelling (Weeks to Months)

The controlled thermal stimulus activates the skin’s wound-healing response. Fibroblasts — the cells responsible for producing dermal extracellular matrix (ECM) components including collagen, elastin, and hyaluronic acid — are stimulated to increase their activity. A 2025 review of monopolar RF histological studies found that dermal collagen content increases both immediately and up to 6 months post-treatment, alongside increases in elastin content and measurable increases in epidermal and dermal thickness.

In a clinical study of 23 Asian subjects who received a single monopolar RF session, significant improvements in skin laxity clinical grades were observed at 4, 12, and 24 weeks post-treatment. Biological elasticity showed significant increase at week 24. Pore count decreased significantly from week 12, and wrinkle appearance began to improve from week 4.

A histometric biopsy study confirmed these findings at a tissue level in patients with Fitzpatrick skin types III and IV — collagen fibre density in the papillary and lower reticular dermis increased significantly at 2 and 6 months post-treatment, with improved coherency of collagen fibre architecture.

Subcutaneous Fat and Body Contouring

At higher energy levels and appropriate settings for body areas, monopolar RF energy heats the subcutaneous fat tissue. The thermal effect on fat cells (adipocytes) contributes to tissue remodelling and fat reduction in the treated zone. A clinical study applying 1 MHz monopolar RF to the lower face and body found that more than 90% of patients showed measurable volumetric reduction by fat reduction at the treated area, with an average reduction of 2.7ml in the targeted zone.

All about Monopolar Radiofrequency

Step-by-step Treatment Process

1. Consultation

The doctor assesses the patient’s skin condition, degree of laxity, anatomy, and goals. The appropriate cartridge, treatment areas, energy settings, and number of passes are planned. For body areas, the target tissue and zone are mapped. Risks, side effects, and realistic expectations are discussed. Informed consent is obtained before treatment proceeds.

2. Preparation

The treatment area is cleansed thoroughly. A return grounding pad is applied to a separate body surface. The treatment zones are assessed and marked. An ultrasound gel or conductive medium is applied to the skin surface to ensure even energy transmission.

3. Treatment

The doctor positions the device handpiece against the skin and delivers focused ultrasound energy in systematic passes across the mapped treatment areas. The doctor selects the appropriate cartridge depth and operating mode for each zone. Patients may feel mild tingling, warmth, or a brief prickling sensation as each pulse is delivered. Treatment speed is approximately 2.5 times faster than the preceding Ultraformer III generation, enabling a typical facial session to be completed in 20–40 minutes.

4. Post-Treatment

The conductive gel is removed. Mild redness and warmth in the treated area are common immediately after the session. A soothing product and sunscreen are applied if the face has been treated. Post-treatment care instructions are provided.

What Results Can You Expect

Immediately after treatment

Mild redness and warmth in the treated areas are common and typically resolve within a few hours. Some patients notice a subtle initial firmness shortly after the session, reflecting the immediate collagen contraction response to heat.

2–4 weeks

Initial improvements in skin texture and early changes in skin firmness may begin to become noticeable. In a published clinical study of 23 Asian subjects, wrinkle appearance began to decrease from week 4 post-treatment.

2–6 months (progressive improvement)

The collagen and elastin remodelling response progresses over time. The same study found that biological skin elasticity showed significant increase at week 24, and pore count decreased significantly from week 12. Histometric biopsy data from a separate study confirmed significant improvements in collagen density and fibre coherency at 2 and 6 months post-treatment.

Number of sessions

Multiple sessions are typically recommended to achieve cumulative improvement. For facial treatments, a course of 3–6 sessions spaced approximately 2–4 weeks apart is commonly recommended, with the number adjusted based on the individual’s skin condition, response, and goals. Body treatment protocols vary by area. Your doctor will advise on the appropriate plan at consultation.

Maintenance

Periodic maintenance sessions may be recommended to sustain improvements as the natural ageing process continues. Your doctor will advise on timing based on your individual response.

Results vary between individuals. The degree and timeline of improvement depends on the skin condition being addressed, treatment settings, number of sessions, the area treated, and lifestyle factors including sun protection and skincare. Results cannot be guaranteed.

Side Effects and Risks

Common, expected side effects

  • Mild redness and warmth at the treatment site (typically resolves within a few hours)
  • Temporary swelling (resolves within 24–48 hours)
  • Sensation of warmth or mild pressure during treatment
  • Temporary sensitivity in treated areas

Less common side effects

  • Prolonged redness beyond 48 hours (more common at higher energy settings)
  • Temporary changes in skin pigmentation (uncommon; risk higher in darker skin types if treatment parameters are not appropriately adjusted)
  • Mild bruising (uncommon)

Rare but reported side effects

  • Burns from uneven energy delivery or settings that are too high for the individual’s skin — the Volnewmer’s automatic energy adjustment and cooling system are designed to minimise this risk
  • Fat atrophy if deep tissue settings are used inappropriately in thin-tissue areas
  • Nerve-related effects (very rare with non-invasive RF at therapeutic levels)

A 2025 clinical study of monopolar RF in 23 Asian subjects reported an average pain score of 0.30 out of 10, and only mild adverse events were recorded with no complications observed. At EHA Clinic, monopolar RF treatments are performed only by trained medical doctors. Full details of potential side effects and risks are discussed at consultation, and informed consent is obtained before treatment proceeds.

Who May Be Suitable for Monopolar Radiofrequency?

Suitable candidates:

  • Adults with mild to moderate skin laxity of the face, neck, or body seeking gradual, non-invasive improvement
  • Individuals concerned about skin firmness and contour in body areas such as the arms, abdomen, thighs, or knees
  • Those seeking a non-invasive, no-downtime approach for skin tightening and body contouring
  • Patients willing to undertake a series of sessions for cumulative improvement
  • Individuals who prefer to avoid injections or devices that involve skin surface disruption
  • Patients of all skin types, as monopolar RF energy acts on tissue water and is not dependent on skin pigmentation
  • Those with realistic expectations about the gradual nature of results over a treatment series

Monopolar Radiofrequency may NOT be suitable for individuals with:

  • Implanted electronic devices (e.g. pacemakers or defibrillators) — monopolar RF requires a return current path through the body, making it contraindicated in patients with implanted electronic devices
  • Implanted metallic hardware or permanent implants in or near the treatment areas
  • Pregnancy or breastfeeding
  • Active skin infections, open wounds, or inflamed skin conditions in the treatment area
  • Active or undiagnosed skin lesions, or suspected skin malignancy in the treatment area
  • Severe skin laxity where non-invasive approaches may be insufficient (surgical options may be more appropriate)
  • Blood-clotting disorders or anticoagulant use (assessed individually)

Suitability is assessed at consultation. The doctor will review the patient’s full medical history, current medications, skin condition, and treatment goals before recommending a treatment plan.

Frequently Asked Questions (FAQs)

What is Monopolar Radiofrequency?

Monopolar Radiofrequency (MRF) is a non-invasive treatment that uses a single-electrode RF handpiece to deliver controlled electromagnetic energy through the skin to heat the dermis and subcutaneous tissue. The current flows from the handpiece, through the body, to a return grounding pad placed on a separate body surface. This allows the energy to penetrate deeper into the target tissue than bipolar RF systems. The thermal effect stimulates collagen contraction and remodelling, leading to gradual improvement in skin firmness and contour over time.

In bipolar RF, the current passes between two electrodes positioned close together on the skin surface — limiting the energy path to the superficial tissue between them. Monopolar RF has a single active electrode (the handpiece) and a distant return pad, allowing the current to travel through a broader tissue depth. RF microneedling (such as Gold RF Microneedling) delivers fractional RF energy directly into the dermis via fine needles, targeting a different depth and pattern. The choice between these modalities depends on the individual’s concerns, skin condition, and treatment goals — discussed at consultation.

For facial treatments, a course of 3–6 sessions spaced approximately 2–4 weeks apart is commonly recommended to achieve cumulative improvement. Body treatment protocols vary by area. The appropriate number of sessions depends on the individual’s skin condition, the area being treated, and the response to initial sessions. Your doctor will advise on the treatment plan at consultation.

Improvements develop progressively over weeks to months as the collagen remodelling response matures. In a published clinical study of Asian subjects, wrinkle appearance began to improve from week 4, pore count decreased significantly from week 12, and biological skin elasticity showed significant increase at week 24 post-treatment. Initial subtle changes in skin firmness may be noticed earlier, reflecting the immediate collagen contraction effect.

Downtime is minimal. Mild redness and warmth are expected after treatment and typically resolve within a few hours. Most patients resume daily activities on the same day.

The device incorporates a vibration and cooling system that operates concurrently with RF energy delivery, designed to manage surface temperature and support comfort during the procedure. A published clinical study of monopolar RF in 23 Asian subjects reported an average pain score of 0.30 out of 10. Patients typically describe the sensation as warmth and mild pressure. Individual experience varies.

Yes. Monopolar RF is applied to both facial and body areas at EHA Clinic. Body treatment areas include the arms, abdomen, thighs, knees, and buttocks. The treatment areas and cartridge selection are assessed and customised at consultation based on the individual’s concerns and anatomy.

Because monopolar RF requires an electrical current to travel through the body between the handpiece and the return pad, it can interfere with or damage implanted electronic devices such as pacemakers and defibrillators. Patients with any implanted electronic device must inform the clinic at consultation, as this is a contraindication for monopolar RF treatment.

Monopolar RF may be used as part of a broader treatment programme alongside injectable treatments, HIFU-based devices, or other skin quality treatments. The combination approach, sequencing, and timing will be discussed with your doctor at consultation based on the individual’s concerns and goals.

All monopolar RF treatments are performed only by registered medical doctors at EHA Clinic, in accordance with the SMC Guidelines on Aesthetic Practices for Doctors and Singapore’s Healthcare Services Act (HCSA) regulatory framework.

In accordance with Singapore’s MOH/HCSA advertising regulations, before-and-after photos are not displayed in public advertising materials. Example images may be viewed in-clinic during consultation for educational purposes, with appropriate clinical context. Results cannot be guaranteed; individual outcomes vary.

Cost depends on the treatment areas, the number of sessions planned, and the cartridge configuration used. A personalised estimate will be provided during the consultation after the doctor has assessed your skin condition and goals.

Post-treatment care

  • Mild redness and warmth are expected and will typically resolve within a few hours; cold compresses may be applied gently if needed
  • Apply broad-spectrum sunscreen (SPF 30 or higher) daily from the day of treatment onwards and minimise direct sun exposure for at least 2 weeks
  • Use gentle, fragrance-free cleansers and moisturisers; avoid active topical ingredients (retinoids, AHAs/BHAs) for 3–5 days or as directed by the doctor
  • Avoid strenuous physical activity, saunas, steam rooms, and prolonged heat exposure for 24–48 hours
  • Do not massage or apply firm pressure to treated facial areas for 24 hours
  • Stay well hydrated; maintain gentle skin hydration during the initial recovery period
  • Attend your scheduled follow-up appointments so the doctor can assess your response and adjust the treatment plan accordingly
  • Contact the clinic if you experience prolonged redness, unexpected swelling, changes in sensation, or any symptoms that concern you
